We're seeking caring, compassionate individuals to join our hospice team in providing care to patients and their families in their time of need. Duties include but are not limited to:

Registered Nurse (RN)

  • Evaluate a patient's physical condition
  • Develop a care plan that addresses any medical needs and social services
  • Coordinate the efforts of interdisciplinary team members to provide appropriate care
  • Perform recertification assessments and complete renewals as required
  • Conduct supervisory visits for LVNs and CHHAs and complete required documentation

Licensed Vocational Nurse (LVN)

  • Follow the plan of care
  • Provide treatments i.e. periodic wound care
  • Order/administer medications
  • Ensure proper documentation of all provided care
  • Work collaboratively with the members of interdisciplinary team to help meet positive patient care outcomes

Master of Social Work (MSW)

  • Assist patients and families in making health care decisions based on personal goals of care
  • Ensure the patient's end-of-life wishes are documented and known by assisting with advance directives, POLST forms
  • Contacting local agencies and/or community resources that may be of help to patients and families (i.e. hospice agencies, Meals-on-Wheels, Life Alert, etc.)
  • Assistance with insurance, Medicare, and Medicaid paperwork
  • Identifying the emotional and spiritual needs of the patient and their loved ones and finding appropriate support as needed
  • Assistance with bridging family gaps to bring loved ones together whenever possible (for example, help obtain temporary Visa's for out-of-country family members)
  • Assisting survivors with necessary arrangements and paperwork after death occurs
  • Assisting survivors in obtaining appropriate grief counseling
  • Identifying other needs of the patient and their support circle and assisting as needed

Certified Home Health Aide (CHHA)

All canidates would need to have:

  • Current CA professional license
  • Current CPR certification
  • Current medical clearance to work (TB, Influenza, physical exam, etc.)
  • Valid CA driver's license and auto insurance
