Community Care & Hospice is a community supported organization providing comprehensive hospice services for those individuals with a serious illness.
Several clinical studies have demonstrated the benefit hospice care can have for patients diagnosed with dementia, cardiopulmonary disease, kidney disease, cancer and other terminal diagnoses.
Community Care & Hospice team members collaborate with patients, families, caregivers, referral sources and other healthcare professionals to provide holistic support for patients throughout Ventura and Greater Los Angeles in any setting (i.e. residential home, nursing facility, group home, hospitals, in-patient units or a patient’s private home). We are always available to answer any questions.
Referral Information
The following questions may help determine whether a patient needs our help.
A patient who:
- May not survive within the year
- Is facing difficult conversations regarding end of life decisions
- Is seriously ill and pain management is needed
- Has more than one chronic condition
- Has been admitted to the ER/hospital several times within six months
As a caregiver:
- How many hours do you spend caregiving?
- Is the amount of care you’re providing overwhelming?
- Are you getting help from friends and family members?
- Have you been ill recently?
- Are you experiencing sleep issues?
- Are you irritable?
- Is the situation taking a toll on your family life or job?
- Is it causing a financial strain?
