For several years, Community Care & Hospice has provided end-of-life care, bereavement services, and caregiver support throughout Ventura and Greater Los Angeles.

• Specialized and complex care delivered for specific illnesses such as heart failure, dementia, and renal disease.
• Home visits by physicians and nurse practitioners. We are happy to work in partnership with your medical team, a practice we have been doing for many years.
